I’ve seen Broly as a super saiyan, and obviously as a Legendary Super Saiyan, but has he ever turned anything else (like Super Saiyan 4, Super Saiyan 3, Super Saiyan 2, or something like Kaio-Ken)

   No he only went legendary super saiyan

   When Broly was killed he could only go LSSJ. He is not a canon character anyway.
